Maddrey’s Discriminant Function

Maddrey’s discriminant function for alcoholic hepatitis suggests which patients with alcoholic hepatitis may have a poor prognosis and benefit from steroid administration. This tool was created by Dr. Willis Maddrey. Dr. Maddrey is professor of internal medicine at The University of Texas Southwestern Medical Center at Dallas. Previously, he directed the liver unit at The Johns Hopkins University School of Medicine and was chairman of the Department of Medicine at Jefferson Medical College.1

In a meta-analysis of 4 controlled trials, we found corticosteroid use to reduce risk of death within 28 days of treatment, but not in the following 6 months. This loss of efficacy over time indicates a need for new therapeutic strategies to improve medium-term outcomes.2
